The lawsuit relates to a Data Security Incident discovered by Reventics on December 15, 2022, where a cybercriminal accessed and encrypted information on its network. An investigation later confirmed that private information was acquired. Defendants notified affected individuals starting in February 2023.
Defendants deny any wrongdoing, and no court has found that it violated any laws.
You are a Settlement Class Member if your personal health information (PHI) and/or personal identifying information (PII) was potentially involved in the Data Security Incident. Notices were previously mailed to affected individuals.
Defendants have agreed to create an $8,150,000 Settlement Fund. If you submit a valid Claim, you may choose to receive either:

DO NOTHING | You will receive no payment and will no longer be able to sue Defendants over the claims resolved in the Settlement. You will remain a member of the Settlement Class and be subject to the terms of the Settlement if approved by the Court. |

EXCLUDE YOURSELF BY JUNE 21, 2025 |
If you ask to be excluded, you will not receive a cash payment, but you may be able to file your own lawsuit against Defendants, for the same claims. This is the only option that leaves you the potential to file your own lawsuit against Defendants for the claims that are being resolved by the Settlement. To be effective, you must submit a request for exclusion by the deadline. |
OBJECT TO THE SETTLEMENT BY JUNE 21, 2025 |
If you do not exclude yourself from the Settlement Class, you may submit an objection explaining to the court the basis for finding the settlement agreement deficient. If your objection is overruled by the judge, you will be bound by the Settlement. |
